WHAT SERVICES HOPE ADHC CAN OFFER?
NURSING SERVICES:
RN and LVN provide nursing care such as first aid, monitoring of blood pressure, blood glucose, weight and medication; and, assist with activities of daily living (eating, dressing, toileting, and walking)
REHABILITATION SERVICES:
Physical, occupational, and speech therapies to improve and maintain ability to function independently
PSYCHOLOGICAL SERVICES:
Consultation and counseling for emotional and mental health issues
SOCIAL SERVICES:
Assistance in problem solving, counseling of personal, family and social issues; and, referral to other health services and community resources
THERAPEUTIC ACTIVITIES:
Recreational and social activities including outings to encourage social interaction, promote stimulation to prevent isolation and deterioration
RESPITE CARE:
Rest for families while participants are at the Center
TRANSPORTATION SERVICES:
Pick up to and from HOPE ADHC
* NUTRITIONAL SERVICES:
Nutritious hot meals and snacks approved by a Registered Dietician.

HOW DO YOU QUALIFY FOR ADHC SERVICES?
ALL of the following program criteria established by the State must be met.
A medical condition that requires treatment or rehabilitative services prescribed by a physician A physical and / or mental impairment that hinders activities of daily living, but not serious so as to require 24- hour institutional care A reasonable expectation that preventive service will maintain or improve the present level of functioning A condition highly potential for further deterioration and institutionalization if ADHC services were not available.
